Employee Benefit Plan

  • HCSA Claim Process

  • If you have selected the Enhanced Health and/or Dental benefit and want to use your HCSA to be reimbursed for your out-of-pocket premium costs, please remember that for now, this must be submitted using a paper claim. Above is the claim form you will need to use – please print and complete as follows:
    • In the top, right-hand corner, select Health Care Spending Account only
    • Complete Part 1 – Plan Member Information
    • In Part 6 – Medical Expenses, simply indicate the dollar amount you are claiming in the white space (as a reminder, you can submit a claim for reimbursement of your health and dental premium costs as often as you’d like, but we’d recommend doing this on either a quarterly or annual basis)
    • Complete Part 8 – Confirmation, Authorization and Signature
    You would then mail the claim form to Great-West Life (now Canada Life) at the address indicated in Part 9 – Submitting Your Claim. Please be sure to include either a copy of your pay stub, or a copy of a letter that I will be delivering to each of you who have selected the enhanced benefit plans, confirming your premium contribution to the health and dental benefit, each time you submit a claim for reimbursement of your premiums. Should you have any questions, please contact Great-West Life (now Canada Life) at their dedicated HCSA service line – 1-877-883-7072.
